About Us:
Center Point DAAC (Center Point DAAC: Leading Addiction Treatment and Counseling in Santa Rosa, CA - Partnership Health Plan Accepted (cpdaac.org)) is expanding and opening a mobile unit to provide Medication Assisted Treatment (MAT) services in Sonoma County and surrounding areas. We're seeking a dedicated Registered Nurse to lead our mobile MAT team and provide high-quality care to individuals with substance use disorders.

Job Summary:
We're looking for an experienced RN to coordinate and deliver MAT services in a mobile healthcare setting. As a Mobile MAT Nurse Coordinator, you'll administer medications, provide nursing care, and collaborate with our interdisciplinary team to ensure comprehensive care. You'll travel to various locations, ensuring compliance with state and federal regulations and providing education to patients and families about substance use disorders and treatment options.

Essential Duties and Responsibilities:
  • Medication management, including administering MAT medications and monitoring patients for efficacy and adverse reactions.
  • Patient care, including comprehensive nursing assessments, developing individualized care plans, and providing education on substance use disorders and harm reduction strategies.
  • Coordination and collaboration with interdisciplinary team members, including physicians, substance abuse counselors, social workers, and peer support specialists.
  • Compliance with federal, state, and local laws, including California-specific regulations related to substance use treatment.
  • Maintenance of accurate and timely electronic health records (EHR) and participation in audits, inspections, and compliance reviews.
  • Mobile clinic operations, including setup and breakdown of mobile clinic sites, maintaining inventory of medications and medical supplies, and adhering to safety protocols.

Qualifications:
  • Graduate of an accredited school of nursing, minimum bachelor’s degree in nursing.
  • Current and unrestricted Registered Nurse (RN) license in California.
  • Minimum of two years of nursing experience, preferably in substance use treatment, behavioral health, or community health settings.
  • Experience with MAT programs highly desirable.
  • Valid California driver's license with a clean driving record.
  • Basic Life Support (BLS) certification required; Advanced Cardiovascular Life Support (ACLS) preferred.

Knowledge, Skills, and Abilities:
  • Comprehensive understanding of substance uses disorders and MAT modalities.
  • Familiarity with California laws and regulations related to substance use treatment and patient confidentiality.
  • Excellent clinical assessment and critical thinking skills.
  • Proficient in EHR systems and accurate documentation.
  • Strong communication and interpersonal skills, with the ability to work effectively in a multidisciplinary team.
  • Ability to work independently in a mobile setting, managing time and resources efficiently.
  • Cultural competency in working with diverse populations, including underserved communities.

Physical Demands:
  • Ability to lift and carry up to 25 pounds.
  • Prolonged periods of standing, walking, or sitting.
  • Ability to travel frequently within the community to various mobile clinic sites.
  • Manual dexterity required for administering medications and operating medical equipment.
  • Ability to work in varying environmental conditions associated with mobile clinic operations.
