Job Title: Outreach Specialist.
Department: MAT Services.
Compensation: $24.00 to $26.00/hour.
Schedule: 40 hrs, Part-time, flexible hours, Monday-Friday.
Reports to: Program Manager.

Center Point DAAC offers rehabilitation and treatment services that interrupt the abusive cycles of psychological, social and economic dislocation by providing critical training and support so that individuals and families can claim self-worth and dignity.

Job Summary:
Center Point DAAC is seeking a compassionate and experienced Outreach Specialist to join our team. As an Outreach Specialist, you will play a vital role in connecting individuals struggling with substance use disorders with our programs and other community resources. Your outreach efforts will focus on the surrounding areas of Santa Rosa in Sonoma County, with the goal of reducing barriers to treatment and promoting recovery.

Key Responsibilities:
  • Conduct educational presentations and outreach activities with local agencies, community providers, and potential participants.
  • Provide educational information about MAT and other available program services.
  • Screen potential participants in various settings, including jails, prisons, and social service providers.
  • Assist clients in developing needs and service plans.
  • Complete accurate, and timely documentation, including charting and progress notes.
  • Coordinate referral services with the coordinator and other team members.
  • Connect clients with community resources, including primary care, mental health services, social services, housing, and vocational services.
  • Assist with follow-up activities and make referrals for continuing care.
  • Provide other services as assigned by the agency.

Qualifications:
  • California State-approved Alcohol and Drug Certification or Registration (preferred).
  • Knowledge of substance use disorders, associated stigma, trauma, and collateral issues (e.g., STDs, HIV/AIDS).
  • 2+ years of experience working in a substance use disorder treatment program (preferred).
  • Professionalism, punctuality, flexibility, and reliability.
  • Integrity to handle sensitive information confidentially.
  • Excellent verbal, written, and interpersonal skills.
  • Understanding of trauma-informed care.
  • Ability to pass and maintain criminal justice security clearance.
  • Valid driver's license and clean driving record.
  • Strong communication, problem-solving, and interpersonal skills.
  • Valid CPR/ First Aid.
  • TB clearance.
  • Bilingual Spanish/English (preferred).
